Samaritan Medical Care Center is moving to a new location at 1580 S. Winchester Blvd. #202, Campbell, CA 95008 on August 16, 2024.
The last day for primary care, urgent care, and occupational medicine service appointments at our current location will be August 15, 2024. Unfortunately, physical therapy services will not be offered in the new location, with services ending June 21.
Here are a few reasons why we are excited about our new location:
Enhanced Facility: We are joining Gateway Family & Urgent Care, which is also part of Vituity and operated by the same local team of doctors. With updated exam rooms and waiting areas, we’re dedicated to providing you with the highest quality of care.
Convenient Access: The new location is approximately 10 miles from our current clinic. Our new location offers convenient parking options and is easily accessible by public transportation, making it simple for you to reach us for your appointments.
Same Warm and Welcoming Team and Environment: Our priority is to create a welcoming and comfortable environment for all of our patients. You can expect the same compassionate team and friendly faces at our new location, ensuring you feel valued and respected during every visit.

Why is the clinic moving?
Our clinic is relocating on 8/16/24. We are joining forces with Gateway Family & Urgent Care and moving to their location at 1580 S. Winchester Blvd, #202 in Campbell, CA. The new location is more modern to provide enhanced patient care.
When is the move happening?
We will provide services at our current location until8/15/24 Beginning 8/16/24, we will begin seeing patients at our new location in Campbell.
Where is the new location?
The new clinic address is 1580 S. Winchester Blvd, #202 in Campbell, CA. It is approximately 10 miles from our current location.
Will the ownership or management of the clinic change?
No, there will be no change in ownership or management. The clinic will continue to be operated by the same physician leadership and team of dedicated healthcare professionals you know and trust.
Will my appointment change because of the move?
No, unless contacted by the staff, we expect all existing appointments will be honored at our new location. You do not need to reschedule.
How can I schedule an appointment at the new location?
You can schedule an appointment the same way you always have, by calling us at (408) 281-2772.
Is there parking available at the new location?
Yes, there is ample parking available for patients at the new location, including designated handicapped parking spaces.
Is the new clinic accessible by public transportation?
Yes, the new clinic location is accessible by public transportation.
Do I need to transfer my medical records to the new location?
No, all medical records will automatically be transferred to the new location securely.
What if I don’t want to transfer my medical records to the new location?
To authorize the release of your medical records, please complete and sign the HIPPA-compliant authorization form to release your medical records. Return the completed form to the clinic as soon as possible but no later than 7/31/24. Our team will need approximately 10 business days to retrieve and prepare your records.
Will the hours of operation change?
Hour hours of operation will not change. We will continue to see patients 7 days a week at the new location.
Will there be a new phone number?
No, the phone number at (408) 281-2772 and website at www.samaritanmedicalcare.com will remain the same.
Will there be any changes to my insurance or billing process?
The move will not affect your insurance or billing process. All existing agreements with insurance providers remain in place.
