Termination of Physical Therapy Services

Samaritan Medical Care Center is moving to a new location at 1580 St. Winchester Blvd, #202 in Campbell, CA on August 16, 2024.

Unfortunately, we will not be offering physical therapy services at the new location. Our last day of offering physical therapy services at our current location will be June 21, 2024.

To find a new physical therapist, please reach out to your insurance provider for guidance to identify a participating provider who meets your preferences and healthcare needs.

To authorize the release of your medical records: We are happy to assist you in transferring your medical records to ensure a smooth transition of care. Medical records are confidential and protected by federal privacy and security regulations. Please complete and sign the enclosed HIPAA-compliant authorization form to release your records. Return the completed form to the clinic as soon as possible. Our team will need approximately ten business days to retrieve and prepare the records.

• You may pick up your medical record(s) at this practice by no later than August 15, 2024; please call in your request at least ten business days in advance of your pickup date.
• You may also have your record(s) sent to you or to a physician you designate.

After the close of the San Jose location, signed requests for copies of medical records can be directed to new location, Samaritan Medical Care Center, 1580 Winchester Blvd #202, Campbell, CA 95008.
